- Three (3) copies of Birth Certificate (1 original, 2 authenticated)
- Passport, Updated Student Visa, ACR Card
- Three (3) Official Transcript of Records (1 original, 2 authenticated)
- Two (2) original copies of Certificate of Good Moral Character (1 from the Dean, 1 from a Professor)
- Three (3) copies of Diploma (1 original copy, 2 authenticated)
- Two ( 2) copies of NBI Clearance
- Five (5) copies of 2×2 most recent photo (black and white)
- One Hundred Dollars ($100.00) for Application Fee. Use international money order or bank draft. (Do not send in CASH)
- A brief narative on how your Medical Education will be financed.
- Original copy of NMAT result not later than 2 years from application.
- Long clear book (orange)
- Php1,500.00 per semester for College Fund.
